You may email me or call to schedule an initial consultation. I would like to know a little about why you are seeking services and general times you are available to see if I’m the right person for you. Please keep in mind that emails are not confidential.
My fee for the 45 minute session is $125, I ask that you pay at the end of the session with cash, check or credit card (MC or Visa). Checks should be made payable to Living-Now, Inc.
I am not on any insurance panels nor do I participate with insurance companies that require pre-certification or treatment plans. If you are seeking psychotherapy and want to use your insurance, I can provide a completed form that you send to your insurance company; and they will reimburse you directly. To find out how much reimbursement you will receive, ask your insurance company what they will pay for a 45-50 minute session of psychotherapy provided by an out-of-network psychologist with a Ph.D.

Once you have reviewed the above forms, please download the following and bring to the first session. Each member of a couple will need to complete these forms. We can discuss any questions you have either before or at your first appointment:

If you are 65 or older, or will turn 65 within the next year, you are required by law to review and sign two copies of the below form indicating that I have "opted-out" of Medicare. Please bring both copies to your first session so that I may sign them. We will each keep a signed copy.
Medicare Opt-Out
If you do not have the capability of downloading these forms, please let me know. You can arrive 20 minutes early for your first appointment. The forms will be in the waiting room on a clipboard with your first name on it.
